Who Should Attend
This three-hour-fifteen-minute (3:15) live virtual training is a free introductory training and is open to SAPC Network providers including:
• Registered and Certified Substance Use Disorder (SUD) Counselors
• LMFTs, LCSWs, LPCCs & Licensed Eligible (LE) AMFTs, ACSWs, APCCs
• Psychologists & Registered Psychological Associates
• Certified Medi-Cal Peer Support Specialists & Peer Support Specialists
• Licensed Vocational Nurses (LVN) & Licensed Psychiatric Technicians (LPT)
• Registered Nurses & Nurse Practitioners

Training Description 
This specialized training will provide a brief review of care coordination service guidelines within the LA County SAPC network and further explore 11 common care coordination needs, such as mental health, housing, and vocational needs. The training will introduce the seven foundational care coordination principles and participants will practice application of these principles through a case example. Through discussion and application over a case example, participants will identify and integrate various cultural factors into care coordination practice. Both traditional and new California Advancing and Innovation Medi-Cal (CalAIM) care coordination models will be discussed and applied to special populations that will benefit from these models. This training will end with discussing ways to improve clinical documentation of care coordination and ethical concerns in care coordination delivery. 
Through group discussion, chat, and polls participants will be engaged throughout the training

Learning Objectives
At the conclusion of the training, participants will be able to:
1. Identify three (3) principles of care coordination and incorporate into practical application.
2. Integrate three (3) cultural factors into care coordination. 
3. Describe and apply two (2) care coordination models to patients in substance use treatment with specific needs. 
4. Implement three (3) methods to improve clinical documentation of care coordination.
