This is a four hour (4) live virtual, free introductory training and is open to SAPC Network 
providers including:

Training Description

This specialized training will breakdown the multiple facets of the care planning process for 
patients with substance use disorders as well as those presenting with complex needs. 
Participants will learn to incorporate new California Advancing and Innovating Medi-Cal 
(CalAIM) documentation requirements in the care planning process. A key component of this 
presentation will focus on developing a clinical mindset by using the ASAM Criteria as a clinical 
framework to guide identifying problems and developing action steps for patients’ substance 
use services and care. Collaboration with the patient, clinical team, and collaterals will be 
discussed in the context of patients’ cultural, linguistic, and individual needs. Lastly, 
participants will have the opportunity to practice various elements of the care planning 
process, including discharge planning, based on a clinical vignette. Participants will develop a 
comprehensive understanding of the core components of effective care planning process 
within the LA County Substance Abuse Prevention and Control’s SUD continuum of care.
Participants will be engaged in group discussion, chat, and polls participants throughout 
training.

 

Learning Objectives

At the conclusion of the training, participants will be able to:
1. Describe the four (4) steps in care planning.
2. Utilize the new CalAIM documentation requirements and the ASAM Criteria to 
identify six (6) problems and formulate six (6) action steps during care planning.
3. Practice documenting care planning in notes based on a clinical vignette. 
4. Discuss three (3) considerations in discharge planning that will support continuity of 
care in the patient’s recovery.
