Case Management/Care Coordination 102

Case Management/Care Coordination 102

Last Friday of the Month Professional Training Series

Case Management / Care Coordination 102 builds on CM/CO 101. It is not necessary to have attended our January 2020 training as we will briefly recap what we learned and build on the basics of case management assessment. If you did attend CM/CO 101, you won’t want to miss this training. This course tackles the SAPC Provider challenge of improving case management and continuity of care for patients receiving drug treatment and co-occurring services in L.A. County. L.A. CADA will train attendees using L.A. CADA’s Case Management Tools to ensure optimum Case Management/Care Coordination assessment and goal setting to create effective Treatment Planning. Interactive break out segments will assist attendees in practicing and fully understanding how to use the information they learn. Facilitators will emphasize the importance of using standardized case management tools, how to document case management, and linking patients to critical healthy living services to sustain recovery.
