
Care Coordination
Last Friday Of the Month Professional Training Series
This new training will focus Counselors and Clinicians on utilizing the six dimensions of the ASAM Criteria as part of a Recovery Oriented System of Care, and as a guide to treatment planning, case management, and care coordination. Practitioners will learn the importance of an initial Treatment Plan following Assessment, and ongoing Treatment Plan development; stabilization of high risk “problem” or “need” areas through counseling, testing and case management (linkage and referrals); transitioning clients through the continuum of care, hopefully into Recovery Support Services, and including the preparation of a Discharge Plan; and retaining and re-engaging resistant clients through discharge prevention techniques.
